File Request Form
This form is for AMUSE employees who wish for the union to request access to their entire employee personnel file from HR on their behalf. The employee personnel file is to include any documents or relevant comments related to their employment per article 16.04 of the AMUSE collective agreement.
Contents of an employee personnel file include
Offer Letters
Paystubs
Probationary Reviews
Evaluations
Disciplinaries (if applicable)
Terminations (if applicable)
Resignations (if applicable)
Legal Documents (will not be pulled by the union)
T4 and RL-1 Tax Slips (will not be pulled by the union)
